Program Summary
Dr. Hein Wellens presented a review of antidromic tachycardias. A must for all electrophysiology fellows and attendings! Dr. Wellens is an exceptional teacher and presents a step by step analysis of intracardiac tracings, focusing on antidromic pre-excited tachycardias. He discussed the differential diagnosis of right sided accessory pathways and atriofascicular fibers.
The retrograde limb is discussed in detail, and the changes in VA intervals with retrograde RBB is also addressed. Carotid sinus massage may terminate an antidromic tachycardia if the retrograde limb is the AV node and will have no effect if the retrograde limb is another accessory pathway.
Dr. Wellens focused on the electrophysiologic characteristics of atriofascicular pathways, and systematically reviewed a series of tracings ullstrating the critical electrophysiologic properties of these pathways. Finally, the characteristics of fasciculo-ventricular fibers were reviewed.
