FHRS Application Information for ABIM- and AOBIM-certified Clinical Cardiac Electrophysiologists

Application Process

• Completed applications from eligible candidates are considered by the Membership Committee.
• Please allow approximately four weeks for the Membership Committee to consider your request.
• Applications can be submitted throughout the year. However, to be considered for fellowship in time for induction at the 2013 Annual Scientific Sessions, your completed application (including letters of support) must be received by the Society no later than 11:59 p.m. [EST] on February 28, 2013. NOTE: The deadline for inclusion at the 2012 Induction has passed.

Application Materials

1. Signed FHRS Application for Physicians Certified by the American Board of Internal Medicine (ABIM) and American Osteopathic Board of Internal Medicine (AOBIM) in Clinical Cardiac Electrophysiology (CCEP).
   
2. Two letters of support from current FHRS members.
3. A letter from you documenting how you meet the criteria below. Please briefly state your intent to apply and information about your CCEP certification.
