Maintaining Affiliate Membership

Fellows who extend training or end one training program and then begin another may be eligible to maintain their Affiliate status. However, members are responsible for keeping the Society informed of their enrollment in a new or extended training.

To ensure uninterrupted benefits, please resub
mit a completed and signed Fellowship Confirmation Form or letter from your training program director which verifies your participation in an eligible program and indicates the month and year that the new/extended fellowship is scheduled to end.

General Information

Complimentary Affiliate memberships are scheduled to conclude one month after completion of training or after a maximum of three years, whichever comes first. Upon conclusion of fellowship training, the Society will automatically convert the membership to the applicable paid membership category — Affiliate members will receive an invoice and must then pay dues to become Physician/Scientist Members or resubmit a
signed Fellowship Confirmation Form or letter from their training program director. Failure to pay dues within 90 days will result in membership suspension.

Affiliates have the same benefits and privileges of physician, scientist, and allied health professionals membership except eligibility to hold office and the right to vote for elected positions.

Affiliate membership is
complimentary and supported
by an educational grant
from Medtronic Corporation.
