Educate Your Patients and Public — Order Your AF Awareness
Resource Kit Today!
The Heart Rhythm Society has created a collection of free resources on Atrial Fibrillation (AF) to provide materials that assist in educating your patients and increase public knowledge about AF risk factors, symptoms and available treatment options. Complete and submit our online order form to receive in-clinic resources, delivered in time for National AF Awareness Month in September!
Your complimentary AF Awareness Resource Kit* will include:
- AF Awareness Campaign Poster (1) — The "A-Fib Feels Like..." campaign creatively illustrates what many patients struggle with as they describe the symptoms of AF. The feeling of a heart that is racing, pounding or fluttering is common, but often difficult to express. Display this 2' x 3' campaign poster in a waiting area, exam room or other highly visible location of your institution to raise awareness
- AF Patient DVD (5) — Designed to help patients and their families understand AF, the multimedia DVD reviews risk factors, symptoms and treatment options, and features patients sharing their real-life experiences
- "A-Fib Feels Like" PSA DVD (1) — Help publicize this important campaign by playing the DVD in your waiting or exam room and including the PSA on the website where you work, on your own blog or your social media profiles.
- "A-Fib Feels Like" Tabletop Posters (1 set of 3) — Reproduce these 8½" x 11" sheets for use as flyers to be distributed at health fairs or other community events, or request high-resolution files for placement in your institution's newsletters or other communications.
- CHADS2 Scorecard (5) — This conveniently sized reference card summarizes the CHADS2 scoring methodology. CHADS2 is a simple, yet clinically valid, prediction rule for estimating the risk of stroke in patients with AF.
- Guide to Atrial Fibrillation (qty. 15) — This brochure includes simple, straightforward descriptions and color illustrations designed to address patients' frequently asked questions (endorsed by the National Stroke Association).
- Heart-shaped stress/squeeze ball (1)
- Managing the Patient with Atrial Fibrillation (5) — Overview of how to manage patients with AF — information on evaluation as well as acute and long-term treatment
- Practical Rate and Rhythm Management of Atrial Fibrillation Pocket Guide — This quick reference pocket guide, updated in January 2010, was adapted from the ACC/AHA/ESC 2006 Guidelines for the Management of Patients with Atrial Fibrillation. It is intended to assist physicians in the diagnosis and treatment of patients with AF.

Identifying Patients at Risk—How to Take Action!
- Assess and Evaluate Your Current Practice
"For patients with AF, understanding the risk for stroke can help determine the most appropriate course of treatment
- Align and Implement a Process Improvement to Identify At-Risk Patients
"Identify tools to address practice gaps (e.g., chart stickers, EMR flags); incorporate the CHADS2 Risk Assessment for all AF patients
- Measure and Sustain Your Process Improvement Progress
Review progress on a monthly or quarterly basis. Are there other opportunities for improvement that can be incorporated into your process workflow?
